Patient Rights
This Notice is provided in accordance with California law (Business and Professions Code § 2290.5 and related provisions) and the Medical Board of California's requirements for healthcare providers.
Carlo P. Honrado, M.D., Inc. is committed to providing every patient with safe, ethical, and compassionate care. The following rights are guaranteed to every patient at our practice.
Right to Courtesy and Respect
You have the right to be treated with courtesy, respect, and dignity at all times.
Right to Know Who Is Providing Care
You have the right to know the name and professional status of any individual providing services to you.
Right to Information About Your Care
You have the right to receive complete information about your diagnosis, treatment, prognosis, and alternatives in language you can understand. You have the right to know the risks, benefits, and expected outcomes of any proposed procedure before consenting.
Right to Informed Consent
You have the right to provide informed consent before any procedure is performed. You may ask questions and request more time before agreeing to any treatment. You may refuse any procedure at any time.
Right to Refuse Treatment
You have the right to refuse any treatment, to the extent permitted by law, and to be informed of the medical consequences of such refusal.
Right to Privacy and Confidentiality
Your medical information is private and confidential. It will not be disclosed without your authorization except as required by law or for treatment, payment, or healthcare operations as described in our HIPAA Notice of Privacy Practices.
Right to Access Your Records
You have the right to access, inspect, and receive a copy of your medical records. A reasonable fee may be charged for copying.
Right to Non-Discrimination
You have the right to care without discrimination based on race, color, national origin, age, disability, sex, sexual orientation, gender identity, religion, or any other protected characteristic.
Right to Language Access
If English is not your primary language, you have the right to receive information in a language you understand, to the extent practicable.
Right to File a Complaint
You have the right to voice grievances and register complaints without fear of retaliation. Complaints may be made directly to our practice or to the Medical Board of California.
Telehealth Services (California BPC § 2290.5)
If telehealth services are offered, you have the right to: (1) be informed the service is being provided via telehealth; (2) verbally consent to the use of telehealth; (3) request an in-person appointment in lieu of telehealth; and (4) access your records from any telehealth interaction.
