HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
§ 1 — Who We Are
Carlo P. Honrado, M.D., Inc. ("the practice," "we," "us") is a HIPAA-covered entity. This Notice describes the privacy practices of our medical practice, including our offices in Beverly Hills and Century City, California, and all staff, physicians, and personnel acting on our behalf.
Note on this website: This website (drhonrado.com) is an informational marketing site only. It does not collect, store, or transmit Protected Health Information (PHI). This Notice applies to PHI created or received in the course of providing your medical care.
§ 2 — How We Use and Disclose Your PHI
We may use and disclose your Protected Health Information for the following purposes without your specific authorization:
Treatment
We may use your PHI to provide, coordinate, or manage your medical care, including sharing information with other physicians, specialists, or providers involved in your care.
Payment
We may use or disclose your PHI to obtain payment for services, including submitting claims to your health insurance carrier.
Healthcare Operations
We may use your PHI for quality assessment, training, accreditation, compliance reviews, and other operations necessary to run our practice.
As Required by Law
We may disclose PHI when required by federal, state, or local law, including to public health authorities, law enforcement, or in response to a court order.
Health Oversight Activities
We may disclose PHI to health oversight agencies for activities such as audits, investigations, and inspections.
Serious Threats to Health or Safety
We may use or disclose PHI to prevent or lessen a serious and imminent threat to the health or safety of a person or the public.
Other uses require your written authorization. This includes most uses of psychotherapy notes, marketing communications, and sale of your PHI. You may revoke an authorization in writing at any time.
§ 3 — Your Rights Regarding Your PHI
- Right to Access: You have the right to inspect and receive a copy of your medical records. We may charge a reasonable fee for copying.
- Right to Amend: You may request that we amend your PHI if you believe it is incorrect or incomplete.
- Right to an Accounting of Disclosures: You may request a list of disclosures we have made of your PHI for purposes other than treatment, payment, or operations.
- Right to Request Restrictions: You may ask us to limit how we use or disclose your PHI. We are not required to agree, except in certain situations (e.g., disclosures to health plans for services you paid for in full out-of-pocket).
- Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location (e.g., only by mail at your home address).
- Right to a Paper Copy: You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.
§ 4 — Our Duties
- We are required by law to maintain the privacy and security of your PHI
- We are required to provide you with this Notice and to follow its terms
- We will notify you in the event of a breach of your unsecured PHI
- We will not use or disclose your PHI without your consent, except as described in this Notice
We reserve the right to change our privacy practices and this Notice at any time. Revised notices will be posted at our offices and on this website.
§ 5 — Complaints
If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
