Carlo P. Honrado, M.D., Inc. ("the practice," "we," "us") is a HIPAA-covered entity. This Notice describes the privacy practices of our medical practice, including our offices in Beverly Hills and Century City, California, and all staff, physicians, and personnel acting on our behalf.

§ 2 — How We Use and Disclose Your PHI

• Right to Amend: You may request that we amend your PHI if you believe it is incorrect or incomplete.
• Right to an Accounting of Disclosures: You may request a list of disclosures we have made of your PHI for purposes other than treatment, payment, or operations.
• Right to Request Restrictions: You may ask us to limit how we use or disclose your PHI. We are not required to agree, except in certain situations (e.g., disclosures to health plans for services you paid for in full out-of-pocket).
• Right to Confidential Communications: You may request that we communicate with you in a specific way or at a specific location (e.g., only by mail at your home address).
• Right to a Paper Copy: You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

§ 4 — Our Duties

• We are required by law to maintain the privacy and security of your PHI
• We are required to provide you with this Notice and to follow its terms
• We will notify you in the event of a breach of your unsecured PHI
• We will not use or disclose your PHI without your consent, except as described in this Notice

We reserve the right to change our privacy practices and this Notice at any time. Revised notices will be posted at our offices and on this website.

§ 5 — Complaints

If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
